Appointment Information

Physicians see patients at two locations, Concord and Davidson offices. To schedule an appointment, please use our main number for either location.

We have a few printed resources to share with our patients. First is a sample letter you will receive as a new patient. Second are two of our standard forms that you can print out and fill in at your convenience before you arrive at the office for your appointment. All of these items are in Adobe PDF format. Routine
Call the clinic during normal office hours to set up an appointment. New patients can usually be scheduled within a couple of weeks, often earlier. We offer same day appointments and have a staff of Triage nurses ready to assist if needed. Please call at least 24 hours before an appointment to reschedule.

First Visit
You will be asked to fill out a complete medical history at the time of your initial appointment. Bring your current insurance card, copay or deductible, medical records, and your prescription bottles. We ask that you arrive 30 minutes prior to your first appointment and then 15 minutes prior to any future appointment. This allows our staff to properly register your visit and update your vitals and medications.

Emergencies
If an emergency arises after hours or on holidays, call CMC - NorthEast at 704.403-3000 or our main number and our physician on call will assist you.

Fees
All of our fees are available upon request. Payment is expected at the time services are rendered. We accept Visa, Mastercard, and American Express. Medicare and most private insurance companies are accepted.
